Clinical/Industrial Placements
At the time of writing, the EHS Branch is about to commence a project to develop guidelines for the student supervisor, work-based learning provider and student. This will be accompanied by a formal agreement and risk assessment template to be completed for all training and practical experience programs within premises or situations controlled by another organisation.
Currently a simple Declaration by External Supervisor form (RTF format, 17 Kb) is to be completed by the supervisor of UTS students who will be working in facilities not controlled by UTS.
